Navigating Dementia NB was a research project that ran from July 2022 to July 2023, piloting a patient navigation program for individuals living with dementia, care partners, and the care team.

Navigating Dementia NB is no longer enrolling participants.

Since July 2022, our pilot project helped over 150 families navigate dementia care in New Brunswick!

We would like to thank all the individuals who participated!

Answer

Patient Navigation is a patient-centered healthcare service delivery model that helps facilitate timely access to care and improve care integration. Patient navigators assist patients, their care partners, and the care team by helping to coordinate patient care; facilitating transitions in care; connecting families with resources; helping families understand the health and social services available to meet their needs; and acting as a resource for the care team.
